- Your separate optional promotional-email preference
- Receive your coverage review request.
- Organize the non-binding review around the information you provide.
- Provide the educational summary connected to the coverage interests you select.
- Send a one-time email receipt for your submission; this is separate from follow-up permission.
- Follow up about your coverage review by email or phone call only when you allow that channel.
- Honor your separate promotional-email choice; promotional messages are not part of the review receipt.
This form collects your name, email address, phone number, state, household size, employment status, coverage interests, budget range, your optional permission for review follow-up by email, your optional permission for review follow-up by phone call, and your separate optional promotional-email preference. Submitting requests a non-binding coverage review and a one-time email receipt; it does not require permission for review follow-up. You may allow email, phone calls, both, or neither for review follow-up. Promotional email is a separate optional choice.
